- When to use
- Use F11.20 when the client meets criteria for Opioid Use Disorder, Moderate. Determine severity by symptom count: mild (2-3), moderate (4-5), severe (6+). Specify current status: in early remission, in sustained remission, or on maintenance therapy.
- Duration
- Early remission: 3-12 months without criteria met (except craving). Sustained remission: 12+ months. On maintenance therapy: receiving agonist medication. Tolerance/withdrawal don't count if on prescribed medication.
- Session codes
- Pair with CPT 90834/90837 for therapy. SBIRT: 99408/99409. Medication management: 99213/99214. Group therapy: 90853. Use 96127 for AUDIT/DAST screening. MAT visits may use specific E/M codes.
- Key indicators
- Impaired control over substance use • Social impairment • Risky use • Pharmacological indicators (tolerance, withdrawal) • Craving • Continued use despite problems • Giving up activities
What F11.20 represents
Moderate OUD represents significant opioid-related problems. Given the opioid crisis, this is one of the most critical diagnoses in current clinical practice.
Key diagnostic criteria
4-5 criteria from the standard substance use disorder framework applied to opioid use. Tolerance and withdrawal don't count toward criteria when occurring under appropriate medical supervision.
Documentation essentials
Document specific opioid(s) used, route of administration, criteria met, and functional impact. Note any medication-assisted treatment.
Differential diagnosis
Distinguish severity levels. Assess for co-occurring pain disorders. Rule out iatrogenic dependence.
Treatment planning
Medication-assisted treatment (MAT) with buprenorphine, methadone, or naltrexone. Combined with behavioral therapy. Harm reduction approaches.
Insurance & billing
Strong acceptance given opioid epidemic. MAT well-supported. Document severity and treatment plan.
Updates & changes
Increased emphasis on MAT as first-line treatment. Regulatory changes for prescribing buprenorphine.
Documentation pitfalls
Don't count tolerance/withdrawal under medical supervision. Document MAT adherence.
