What F25.1 represents
The depressive type of schizoaffective disorder involves concurrent psychotic and depressive symptoms, plus periods of psychosis without mood symptoms. No manic episodes have occurred.
Key diagnostic criteria
- Same criteria as schizoaffective disorder generally
- Only depressive episodes (no manic episodes) are part of the presentation
- Depressed mood must be prominent during concurrent psychotic periods
Documentation essentials
Document depressive episodes, psychotic symptoms, and periods of psychosis without mood symptoms. Track longitudinally.
Differential diagnosis
Distinguish from MDD with psychotic features (no independent psychosis) and schizophrenia (no prominent mood episodes).
Treatment planning
Antipsychotic + antidepressant. Avoid mood stabilizers unless indicated. Psychosocial interventions.
Insurance & billing
Accepted code. Document both symptom domains and longitudinal course.
Updates & changes
DSM-5 clarified the lifetime duration requirement for mood episodes.
Documentation pitfalls
This is a longitudinal diagnosis requiring careful tracking of mood and psychotic symptoms over time.
